*All reunion attendees (including kids 12+), students, faculty, staff, and postdoctoral scholars and their guests coming to campus this fall will be required to be fully vaccinated for COVID-19. The time to apply for an exemption for religious or medical reasons has passed. Children between the ages of 2-12 can attend if they show proof of a negative test taken with 72 hours of arriving on campus. Children under the age of 2 do not require vaccination or proof of a negative test. This requirement is based on our COVID-19 Vaccine Governance Committee’s review of conditions and developments around vaccination against the virus. You and your guest (if you are bringing one) will also be required to follow the current mask mandates as specified by Santa Clara County.

**You may use your original vaccination record card from the CDC, or a digital “SMART Health Card” as proof of vaccination along with your driver’s license or photo ID. Your proof of vaccination must include the following:

Full name

Birthdate

Vaccine name/manufacturer

Date(s) of vaccination
